Library Card Registration

If you are NOT a Haverhill resident, student or taxpayer, please come to the library to get a 30-day temporary card (MVLC residents) or a fee card (NH residents). Applicants less than twelve years of age must have a parent or guardian sign their application and accept responsibility for them, so for child applicants please print this form out using your browser’s print feature, and either mail, fax or deliver this application form to Haverhill Public Library (mail and fax info will appear on the printed form).

Failure to return overdue library materials; failure to report lost library materials; destroying or damaging library materials; or failure to reimburse for such lost materials or damages is a violation of M.G.L. Chapter 61, Acts of 1990, M.G.L. Chapter 266, Sections 99 and 100 and is punishable by a fine of up to $25,000.
