Library Card Registration

Haverhill residents, students, and taxpayers may use this form to apply for a library card. Residents of the service area of another MVLC Library (see full list here) may get a 30-day temporary card only by coming to the library. At this time, Haverhill Public Library is unable to provide cards to out-of-state residents.
Applicants less than twelve years of age can apply online, but must have a parent or guardian accompany them to the library to pick up their library card at the Main Desk.  All other applicants will have their cards mailed to them.
